Dear Patient,

Welcome to our practice! Thank you for allowing us to serve your rheumatology needs and we look forward to your visit. The following information is to allow for a smooth transition into our practice.

Please arrive at least 30 minutes prior to your appointment time to allow us sufficient time to process your paperwork.

To expedite our check in process, please complete the new patient paperwork prior to your appointment. When you arrive at our office, please present your completed paperwork, photo ID, and your insurance cards. If your insurance plan requires a referral, it is your responsibility to contact your primary care provider and ensure they have forwarded our office a valid referral. We may not be able to see you if a referral is not on file with our office by your scheduled appointment date.

At Rheumatology & Autoimmune Specialists, your first appointment includes a thorough exam and gathering any information that Dr. Tareen must be aware of in order to provide care specifically to your health. We understand that all patients are different and require individualized care.

At our practice, every patient can expect to be treated with prompt service at their specified appointment time. Our patients can also look forward to a comfortable experience. We understand that visiting a new doctor can be stressful, which is why our practice works actively to accommodate all patients and alleviate any fears they may have about new treatments. Familiarizing yourself with our practice, services, and our doctor is a great first step to creating a long term and comfortable experience.


Office Policies

All non emergent patient messages and phone calls will be answered in 24-48 business hours. Repeated messages will not be expedited.
If there is an emergency then we recommend to call 911.
